NOTE: If you have already completed one of these forms and have nothing to add at this, then it is not necessary for you to complete a second form.  HOWEVER, some of the questions have changed since we sent it out the last time.  Please read them carefully.

  Yes, the idea of a Swigart - Van Dyke reunion interests me very much! 
  I plan to attend and will bring about          additional people. 
  I will not be able to attend, but I think it is a great idea.  Go for it!
  I would be willing to help on a committee for this year’s reunion. 
  I would be willing to help subsidize the reunion beyond the registration fee. 
  I have family history items I think would be interesting to have on display. 
  I am a genealogy buff and have information about the family. 
Activities for children or high school youth would be helpful for

  High School Youth interested in helping supervise younger.
  We would like to have child/youth activities available during the banquet.
  We plan to have our children/youth attend the banquet with us.
